IMPORTANT NOTES BEFORE YOU APPLY ONLINE:

• The online application has five (5) sections. All five sections must be completed fully in one sitting, which takes approximately twenty minutes.

• To complete Section #5, you will need the two (2) documents below, and to upload them as PDF files or as photographs from a smartphone or camera:

1. Photo of Drivers License (Class A or B)

2. 10-Year Motor Vehicle Report (Current MVR)
